Archaeologists unearth Tuscaloosa's early history

June 26, 2013 ? University of Alabama archaeologists are getting a glimpse of what life in Tuscaloosa might have been like more than 180 years ago. From bottles and porcelain pieces to soil and flotation samples taken from privies, or outhouses, the analysts are discovering many "stories" of Tuscaloosa's past.

For the past two months, UA's Office of Archaeological Research has been analyzing artifacts found at the former City Fest lot, located on the corner of University Boulevard and Greensboro Avenue. The University was contracted by the City of Tuscaloosa to perform an archaeological investigation per federal guidelines in preparation for construction of a new Embassy Suites hotel.

Beginning in January, project director Brandon Thompson and his team began investigating the "Bank of the State site." In February, they stripped the remaining parking lot and exposed some "incredible" features, including many foundation remains from buildings that date back to before 1820, said Matt Gage, director of the Office of Archaeological Research.

Initial occupation of the site dates to 1816 when Revolutionary War veteran John Click built a log cabin on the property. However, he never got a deed to the property and lost it to John McKee in 1823. McKee was the Cherokee, Chickasaw and Choctaw agent at the time, as well as a land surveyor, and he had helped lay out the city of Tuscaloosa, Gage said.

Over the years, the property was home to numerous businesses, including Augustin Lynch's cabinet manufactory. Known as one of the most important Antebellum furniture makers of the time, Gage said Lynch provided furniture for the Capitol building -- at the time located only a few blocks to the west -- and for some of the early University of Alabama buildings.

He also created ivory billiard balls and sold them to people in Washington, D.C. Gage said they discovered ivory on the site, as well as rusted tools such as saw blades and drills.

The Bank of the State was built on the property in 1829, and Gage said they found some beautiful decorative pieces from that building, as well as a few Spanish coins. The coins are reales minted in Brazil, Guatemala City and Mexico City. The coins were found in pits containing British gun flints and early bottles closer to where Click's log cabin had been, so Gage predicted they were either associated with traders coming through Tuscaloosa or early dealings with the Bank of the State.

"In the early 1800s, foreign currency was used as frequently as coinage minted in the country," he said. "There were so few mints in the U.S. at the time, any currency of monetary value made of gold, silver or copper was given value and could be exchanged as easily as currency minted in the U.S."

The property also housed an ice factory, numerous shanties and other dwellings, a hotel and the Drish building, which was initially used as a warehouse and then a Civil War prisoner-of-war facility. Artifacts discovered included various bottles (including those that held food, as well as drink and medicine), buttons, porcelain pieces, printing press letters, early smoking pipes, architectural elements from the buildings and more.

A gold mine for archaeologists when it comes to historical sites are wells and privies, said Gage, and they found several on this site, including some that had been used by the Union soldiers housed at the Civil War prison. Using soil and flotation samples from the privies, analysts can determine everything from what individuals were eating to how they were being treated, he added.

"It's just a wealth of information," Gage said.

"Tuscaloosa has a very rich history. When you think of the early history of Tuscaloosa, even though the state capitol was here, you still figure that it was a small little enclave, people going about their lives with a predominant lifestyle involving agriculture, but commerce is a major aspect of this block," Gage said.

"There are so many elements of this site that provide a fantastic glimpse of the past and knowing that past is incredibly important. You can never know who or what you are without knowing your history."

Sports such as ice hockey and soccer accounted for more than half the injuries, said Dr. Gabreila Ilie, lead author of the study and a post-doctoral fellow at St. Michael's Hospital.

Traumatic brain injuries, such as concussions, were reported more often by males than females, by those with lower school grades and by those who used alcohol or cannabis in the previous 12 months, she said.

The study is published today in the Journal of the American Medical Association.

Dr. Ilie said this is one of the first studies of traumatic brain injury to focus only on adolescents and to include all of their self-reported TBIs. Most previous studies were based their reporting only on hospital records. Concussion is the most common form of traumatic brain injury.

The data used in the study were from the 2011 Ontario Student Drug Use and Health Survey (OSDUHS) developed by the Centre for Addiction and Mental Health (CAMH). The survey, one of the longest ongoing school surveys in the world, contains responses from almost 9,000 students from Grades 7-12 in publicly funded schools across Ontario. The OSDUHS began as a drug use survey, but is now a broader study of adolescent health and well-being. For the first time in 2011, questions about traumatic brain injury were added to the survey.

"The questions about TBI were added to the OSDUHS because there were no current data on prevalence in the adolescent population," said Dr. Robert Mann, a senior scientist at CAMH and director of the OSDUHS.

"Early research has indicated that there may be links between TBIs and mental health and substance use during adolescence we plan to study this in the near future."

The survey found that 20 per cent of adolescents in Ontario said they had had a traumatic brain injury in their lifetime. It found that 5.6 per cent of them had had such an injury in the past 12 months.

Dr. Ilie said this suggests the prevalence of TBI among young people is much higher than previously known, because many head injuries remain uncounted when they are not being reported to parents, teachers, sports coaches or health care workers. In Canada, 50 per cent of all injuries that kill and disable youth involve a TBI.

This new research found that 46.9 per cent of the TBIs reported by adolescent females occurred during sports (e.g., hockey, skate boarding); the figure was 63.5 per cent for males.

Students who reported drinking alcohol occasionally/frequently and those who reported using cannabis 10 or more times over the past 12 months had more than five times and more than three times the odds, respectively, of acquiring a traumatic brain injury in the past 12 months than students who reported abstinence. The survey also showed that students who reported overall poor grades at school (below 60 per cent) had almost four times the odds of a lifetime acquired brain injury than students who reported grades at or above 90 per cent.

"Traumatic brain injury is preventable," said Dr. Ilie. "If we know who is more vulnerable, when and how these injuries are occurring, we can talk to students, coaches, and parents about it. We can take preventive action and find viable solutions to reduce their occurrence and long-term effects."

Brain injuries among adolescents are particularly concerning because their brains are still developing. There is growing evidence that people who have had one or more concussions are at greater risk of future concussions, and evidence that multiple brain injuries can result in lasting cognitive impairment, substance use, mental health and physical health harms.

This study is part of a team project grant awarded to Dr. Michael Cusimano, a neurosurgeon and concussion researcher at St. Michael's by the Canadian Institutes for Health Research and the Ontario Neurotrauma Foundation. NEW YORK (AP) ? Broadway theaters will dim their marquee lights Wednesday night in memory of James Gandolfini, "The Sopranos" star who earned a Tony Award nomination in 2009.

The Broadway League said Tuesday the lights will be dimmed for one minute at exactly 8 p.m. EDT.

The 51-year-old actor died Wednesday in Rome. A funeral will be held Thursday at the Cathedral Church of Saint John the Divine in New York City.

Gandolfini received a Tony nomination for his role in the award-winning "God of Carnage." He also appeared on Broadway in "On the Waterfront" in 1995 and "A Streetcar Named Desire" in 1992.

Said Charlotte St. Martin, executive director of the League: "Gandolfini was a consummate actor who brought individuality to each role and inspired a true connection with the audience."

Students' device aims to avert repeated breast cancer surgeries

June 25, 2013 ? When a breast tumor is detected, many women opt to have a lumpectomy, which is surgery designed to remove the diseased tissue while preserving the breast. But during this procedure, doctors cannot learn right away whether all of the cancerous tissue has been removed, with no microscopic signs that cancer cells were left behind. Because of this delay, one in five of these women -- up to 66,000 patients annually in the U.S. alone -- must return for a second surgery to remove remaining cancer. These follow-up operations boost healthcare costs and can lead to delays in receiving other treatments such as radiation and chemotherapy.

To reduce the need for these second surgeries, four Johns Hopkins graduate students have designed a device to allow pathologists to quickly inspect excised breast tissue within 20 minutes, while the patient is still in the operating room. If this inspection indicates that the tumor was not fully removed, additional tissue can then be removed during the same operation. Eliminating the need for a second operation could also curb some of the additional anxiety these patients face.

The device is still in its prototype stage, but the students say their goal is to give breast cancer patients the same rapid review that commonly occurs when tumors are removed from elsewhere in the body. The students learned about the second-surgery dilemma while observing medical procedures last summer as part of a year-long biomedical engineering master's degree program. In this program, students learn to design new medical tools and products that address urgent healthcare needs.

"We spoke to breast cancer surgeons," said Hector Neira of Silver Spring, Md., one of the student inventors. "They told us that they are desperate for something that will allow them to remove the tumor in its entirety the first time, so that the patient doesn't have to come back for a second surgery."

So far, the team's system has been tested on animal tissue and human breast samples from a tissue bank, but it has not yet been used on patients. Over the past year, however, the students' device design and market analysis have earned them more than $40,000 in college business plan prize money. In the recent Design Day event for Johns Hopkins biomedical engineering students, the team received the top People's Choice award. And, although all of the student inventors received their master's diplomas in May, two have received funding to remain at Johns Hopkins and continue refining the project over the coming year.

Although the financial support and recognition is helpful, "that's not our ultimate goal," said Anjana Sinha of Princeton, N.J., another one of the student inventors. "We're not doing it for the money. We want to improve healthcare practices and raise the standard of care for these breast cancer patients. Why can't they get the same type of quick results that people with other types of cancer receive?"

When most tumors, such as those in the liver, are removed, the pathology staff can quickly flash-freeze the tissue and slice off paper-thin samples for microscopic examination. If the pathologist sees that cancer cells extend to the outer edge or margin of a sample, the surgeon is advised to remove more tissue from the patient. But breast tissue poses a problem: it possesses a high fat content and does not freeze well, causing the samples to smear, form gaps and become unsuitable for a quick review. Instead, breast tissue must be preserved and analyzed in a more time-consuming process that requires the patient to return to the operating room if the first surgery appears to have left cancer cells behind.

To solve this problem, the graduate students brainstormed for an engineering solution. Their most promising and practical idea was a device that applies an adhesive film to the breast tissue before it is sliced. The film holds the delicate tissue together, preventing damage to the samples during the slicing process. The result, the students said, is a sample that can be clearly reviewed by a pathologist within 20 minutes of its removal, potentially eliminating the need for a second operation on another day.

The low-cost system includes a reusable applicator and a proprietary disposable film. The students said the need for their product is significant, citing the estimated 330,000 lumpectomies that are performed annually in the United States alone.

"I think the students have been incredibly creative in their development of this concept, and they are addressing a very real need in the field of breast cancer surgery," said Melissa Camp, a Johns Hopkins assistant professor of surgery who worked with the team. "Accurate assessment of margin status during the initial operation will lead to fewer re-operations, and this will be beneficial for patients in many respects. I look forward to their continued work!"

At Johns Hopkins, the pathology device was developed under the supervision of the university's Center for Bioengineering Innovation and Design. The center teams students with faculty researchers, physicians and others who help them understand healthcare needs and guide them as they propose solutions, then build and test prototypes. CBID operates within the Department of Biomedical Engineering, which is shared by the university's School of Medicine and its Whiting School of Engineering.

Along with Neira and Sinha, the student inventors of the breast cancer device were Qing Xiang Yee of Singapore and Vaishakhi Mayya of India. Sinha and Mayya will remain at Johns Hopkins during the coming year to continue working on the project with David Shin of Seattle, another recent graduate of the CBID master's program. The students also will continue collaborating with advisers from the School of Medicine, including Ashley Cimino-Mathews, an assistant professor of surgical pathology, and James Shin, a surgical pathology research specialist. Jason Benkoski, a senior materials scientist from the Johns Hopkins Applied Physics Laboratory, will serve as a technical adviser to the team. The Wallace H. Coulter Foundation is providing funding for these students to continue working on the project this year.
